L’auto-observation des activités quotidiennes s’accompagne-t-elle d’une amélioration de la symptomatologie dépressive ? Does the self-monitoring of daily activities go hand in hand with an improvement of depressive symptomatology?

Le premier objectif de l’étude était de déterminer si l’auto-observation des activités quotidiennes accompagnée d’une évaluation des affects associés a un impact sur la symptomatologie dépressive, l’activation comportementale et le ressassement. Le deuxième objectif était de comprendre le rôle de la conscience émotionnelle dans ces effets. Trente huit participants de la population générale ont été répartis aléatoirement entre le groupe contrôle (protocole d’activation comportementale) et le groupe expérimental (protocole d’activation comportementale accompagné d’un relevé des affects). L’ensemble des participants a rempli avant et après une intervention d’activation comportementale (relever ses activités quotidiennes durant deux semaines) des questionnaires évaluant la symptomatologie dépressive, l’activation comportementale, le ressassement et la conscience émotionnelle. Seuls les participants du groupe expérimental évaluaient les niveaux d’importance, de plaisir et de maitrise ressentis pour chaque activité. Les participants du groupe expérimental, mais pas ceux du groupe contrôle, ont amélioré leur niveau de conscience émotionnelle. De plus, ils ont rapporté ressentir plus de plaisir dans leurs activités à la fin de l’intervention. Aucune différence n’a émergé entre les deux groupes pour les autres variables. Indépendamment du groupe, les participants ont rapporté moins de symptômes dépressifs, plus d’activation et moins de ressassement après l’intervention. Les résultats suggèrent que l’auto-observation des activités quotidiennes a des effets bénéfiques. L’utilisation de cet outil en psychothérapie est discutée.

The first purpose of this study was to determine if self-monitoring of daily activities accompanied by a self-evaluation of associated affects has an impact on depressive symptomatology, behavioural activation and brooding. The second purpose was to understand the role of emotional awareness in those effects.  Thirty-eight participants of the general community were divided randomly between the control (protocol of behavioural activation) and the experimental (protocol of behavioural activation with affects evaluation) groups. All participants fill in forms evaluating depressive symptomatology, behavioural activation, brooding and emotional awareness before and after a behavioural activation intervention (reporting their daily activities during two weeks). Participants from the experimental group only evaluated the level of importance, pleasure and control felt during each activity. The participants from the experimental group, but not those from the control group, improved their level of emotional awareness. Moreover, they reported more pleasure felt during activities at the end of the intervention. No difference emerged between the two groups regarding the other variables. Independently of the group, participants reported less depressive symptoms, more activation and less brooding after the intervention. Results suggest that the self-monitoring of daily activities has good effects. The use of these tools in psychotherapy is discussed.

Prévenir la dépression au secondaire : Évaluation des processus d’un programme probant en contexte de pratique courante Preventing depression in secondary school students: a process evaluation of a brief indicated cognitive-behavioral group depression program in everyday practice

 

Plusieurs interventions ont été développées pour prévenir les difficultés psychologiques chez les jeunes, mais peu d’entre elles ont été testées dans un contexte de pratique courante. Dans cet article, nous rapportons les résultats d’une évaluation des processus du programme cognitivo-comportemental Blues visant à prévenir la dépression chez les jeunes que nous avons examiné en contexte réel dans trois écoles secondaires montréalaises. Nos objectifs étaient de décrire la mise en œuvre du programme, d’identifier les facteurs ayant facilité ou entravé cette mise en œuvre et de déterminer si la qualité de la mise en œuvre du programme était associée à ses effets chez les participants. Nous avons colligé des données qualitatives et quantitatives auprès de 37 jeunes et de 6 intervenants qui ont participé au programme. Les résultats montrent que le programme a pu être mis en œuvre adéquatement sur la plupart des indicateurs. Par contre, les taux relatifs aux pourcentages de devoirs réalisés et la tenue des séances de rattrapage se sont avérés plus faibles que prévu et les ateliers, plus longs. Le principal facteur facilitant identifié par les jeunes concerne l’utilité du programme. Par contre, pour les intervenants, il s’agit de son caractère clés-en-mains. Le principal obstacle identifié par les jeunes concerne la difficulté à faire les devoirs et leur niveau de difficulté tandis que les intervenants mentionnent le manque de temps pour animer les groupes. Enfin, nos résultats suggèrent que les jeunes qui ont participé à un plus grand nombre de séances ont plus bénéficié du programme. Notre étude permet d’articuler plusieurs recommandations qui seront utiles pour soutenir les efforts de diffusion du programme.

 

 

Several interventions were developed in order to prevent psychological difficulties among adolescents, but few of these interventions were tested in a standard practical context. In this article, we report the results of the process analysis of the Blues program, a cognitive behavioural program aimed at preventing depression among adolescents, which we examined in the actual context, in three high schools in Montreal.  Our objectives were to describe the implementation of the program, to identify the factors which facilitate or hamper this implementation and to determine whether the quality of the implementation was associated to its effects among the participants.  In order to do so, we compiled mixed data among 37 adolescents and 6 mental health professionals who participated in the program. The results demonstrated that the program was adequately implemented for most of the indicators.  However, the rates related to the completion of homework and catch-up sessions were weaker than predicted and the workshops longer. The principal facilitating factors which stand out are the usefulness of the program (adolescents) and its turnkey character (mental health professionals). The principal obstacles are the lack of time (mental health professionals) and the difficulty and limited interest when it came to completing homework (adolescents). Finally, our results suggest that the adolescents who participated in a greater number of sessions benefited more from the program. Our study enables us to formulate several recommendations which will be useful to support the diffusion efforts of the program.

Introduction : Bien que le modèle transthéorique de changement (MTC) soit reconnu auprès de gens ayant des problématiques comportementales, peu de données sont disponibles sur son applicabilité auprès de clients ayant un trouble anxieux ou dépressif. Objectif : L’objectif de cette étude est de vérifier l’apport du MTC dans cette clientèle en explorant les relations entre la motivation au changement, l’alliance thérapeutique et les effets de la thérapie. Méthode : Les participants (N = 131) se sont présentés volontairement à une clinique universitaire de psychologie afin de s’engager dans une psychothérapie cognitivo-comportementale pour des symptômes anxieux ou dépressifs. Des questionnaires ont été remplis en début et en fin de traitement. Résultats : La majorité des clients débute la thérapie dans le stade de contemplation. Les clients plus engagés dans l’action et moins déprimés évaluent l’alliance plus favorablement alors qu’aucune variable n’est associée à l’alliance évaluée par le thérapeute. Aucun lien n’est observé entre la motivation au changement et les effets de la thérapie sur les symptômes. Conclusion : Les résultats suggèrent un apport intéressant, mais aussi limité du MTC auprès d’une clientèle anxieuse ou dépressive. La motivation au changement est associée à l’alliance thérapeutique selon la perspective du client. L’importance pour le clinicien d’évaluer la perception du client de la relation et d’adresser l’ambivalence face au changement est soulevée. Étant donné que plus de 75 % des clients débutent la thérapie dans le stade de contemplation, la pertinence d’utiliser l’entretien motivationnel en début de traitement est discutée.

 

Introduction: Although the transtheoretical model of change (TMC) is supported for people with behavioural problems, few data are available on its application for patients with anxiety or depressive disorders. Aim: The aim of this study is to verify the contribution of TMC in this population by exploring the relationships between motivation to change, therapeutic alliance, and therapy’s effect on symptoms. Method: Participants (N = 131) presented themselves voluntarily to the university’s clinic of psychology to begin a cognitive-behavioural therapy. Questionnaires were filled at the beginning and at the end of the treatment. Results: The majority of clients begin therapy at the contemplation stage. The most engaged in action, and less depressed clients, evaluate more positively alliance whereas no variable is associated with alliance as evaluated by the therapist. No link is observed between motivation to change and therapeutic effect on symptoms. Conclusion: Results suggest an interesting, but limited, contribution of the model in an anxious or depressed population. Motivation to change is associated with therapeutic alliance according to the client’s perspective. The importance for the therapist to evaluate the client’s perception of the relationship and to address ambivalence to change is raised. Given that more than 75% of clients begin therapy in the contemplation stage, the usefulness of using Motivational Interviewing at the beginning of treatment is discussed.

Cet article décrit un projet d’échange et de transfert des connaissances visant à adapter et à diffuser la thérapie cognitivo-comportementale (TCC) pour l'insomnie au contexte du traumatisme craniocérébral (TC). L’article présente d’abord les spécificités liées à la clientèle qui ont mené aux adaptations du matériel thérapeutique, notamment les besoins en termes de gestion de la fatigue qui accompagne souvent l’insomnie. Suit un survol du contenu et des évidences d’efficacité des interventions cognitivo-comportementales pour l’insomnie et pour la fatigue, dans la population générale et plus spécifiquement dans la population cébérolésée. L’article présente ensuite les démarches entreprises pour guider le processus d’adaptation du protocole de TCC d’origine, entre autres les collectes d’information pour comprendre les besoins des intervenants, l’intégration des savoirs tacites des cliniciens d’expérience (particulièrement par rapport à la fatigue), et la phase test du matériel avec des cliniciens-champions. Plusieurs leçons ont été apprises à chacune des étapes du processus et ont guidé l’adaptation et la mise au point du matériel: par exemple le format du protocole testé empiriquement est présenté de manière plus flexible (modules indépendants pour chacune des stratégies thérapeutiques plutôt qu’un format séance par séance), des versions simplifiées de certains outils d’évaluation ou d’intervention ont été ajoutés pour accommoder des clients avec des limites sur le plan cognitif, et des recommandations spécifiques à la clientèle ont été incorporées (ex. limitations physiques, aspects motivationnels, comorbidité avec la psychopathologie). La dernière partie de l’article présente de manière plus détaillée les adaptations effectuées.

 

This article describes a knowledge transfer and exchange project which aimed to adapt and disseminate cognitive behavioural therapy (CBT) for insomnia adapted to the context of traumatic brain injury (TBI). First, we present the specificities of the TBI population and how these have led to the adaptation of our evaluation and therapeutic tools, particularly regarding  fatigue which is a major issue in this population. We then provide an overview of the evidence demonstrating the effectiveness of CBT for insomnia and fatigue, within the general population and more specifically for individuals suffering from TBI. The article then describes the steps taken to guide the adaptation of the original CBT protocol. These steps included: gathering information to understand practitioners’ needs, integration of tacit knowledge of experienced clinicians (particularly regarding fatigue), and testing the material with qualified clinicians. We present lessons learned during each stage of the process: for example, increased flexibility in administrating the CBT protocol (independent modules for each of the therapeutic strategies rather than using a session-by-session format), simplified versions of assessment or intervention tools to accommodate patients with cognitive limitations, and specific recommendations linked to physical limitations, motivational aspects, and comorbidities psychopathology. The last section of the article provides a more detailed description of the adaptations implemented.

Le trouble panique (TP) est l’un des troubles psychiatriques les plus souvent rencontrés chez les patients souffrant de maladie coronarienne athérosclérotique (MCAS). L’application du traitement psychologique de premier recours pour le TP, soit la thérapie cognitivo-comportementale (TCC), peut être complexe chez les clients souffrant de MCAS. Les défis potentiels concernent notamment la gestion du risque médical et la similarité dans la présentation des symptômes des deux conditions. En se basant sur la littérature dans le domaine, le présent article propose plusieurs adaptations à la TCC du TP qui peuvent être mises en place pour favoriser son acceptabilité et son efficacité chez les clients souffrant également de MCAS.

 

Panic disorder (PD) is one of the most common psychiatric disorders in patients with coronary artery disease (CAD). The application of the first-line psychological intervention for PD, cognitive-behavioral therapy (CBT), may be complex in patients suffering from CAD. Potential challenges include medical risk management and symptom similarities between both conditions. Based on the literature in the field, the present paper proposes a series of adaptations to CBT for PD that can be implemented in order to promote its acceptability and success in clients suffering from PD and CAD.

L’expérience des thérapeutes assistant les utilisateurs d’un auto-traitement guidé en ligne pour promouvoir la résilience après une catastrophe naturelle Therapists' experience in assisting users of an online guided self-treatment to promote resilience after a natural disaster

Contexte. Malgré les nombreux avantages de la télésanté mentale, les interventions en ligne sont encore sous-exploitées. Une des raisons provient des préoccupations des cliniciens et prestataires de soins quant à: (1) la capacité à forger l’alliance thérapeutique, (2) la capacité à communiquer les émotions et l’empathie, et (3) la pertinence de la télésanté mentale pour un certain type de clientèle.Objectifs. La présente étude vise à explorer l’expérience des thérapeutes assistant les utilisateurs d’un auto-traitement en ligne selon ces trois domaines de préoccupation.Méthode. La plateforme RESILIENT a été développée pour promouvoir la résilience après une catastrophe naturelle. Elle donne accès gratuitement aux personnes sinistrées à un programme de 12 séances visant l’amélioration des symptômes de stress post-traumatique, d’insomnie et de dépression. Les données ont été récoltées à l’aide d’entretiens focalisés avec les thérapeutes ayant participé à une étude d’efficacité de la plateforme, et analysées à l’aide d’une analyse qualitative thématique.Résultats. Les thérapeutes ont été agréablement surprises par la facilité avec laquelle elles ont développé une forte alliance avec leurs patients et le fait que la plateforme en ligne n’entravait pas leur capacité à percevoir les émotions des patients et à ressentir de l’empathie envers eux. Elles ont identifié des avantages (p.ex. s’y sentir plus à l’aise qu’en présentiel) et désavantages (p.ex. absence de signaux non-verbaux), uniques à cette forme de thérapie. Finalement, la pertinence de l’intervention a été évaluée comme étant moindre chez les patients ayant un profil symptomatique plus sévère et des attentes ne correspondant pas à l’intervention. L’engagement du patient au sein du programme semblait être un facilitateur important ayant un impact sur l’alliance, la communication et, ultimement, l’efficacité du traitement.Conclusion. Nous suggérons plusieurs approches pour améliorer les interventions en ligne en accordant une importance particulière aux avantages uniques de ce type d’intervention. Nous proposons également d'intégrer les connaissances et les compétences spécifiques aux interventions en ligne dans la formation des cliniciens et autres prestataires de soins afin d'améliorer les attitudes vis-à-vis de la télésanté mentale et la capacité de les utiliser plus efficacement, pour plus de problématiques et dans un plus large éventail de contextes de soins de santé mentale.

 

Background. Despite the many advantages of telemental health, online interventions are still underutilized. One major reason lies in the providers and clinicians’ shared concern about the (1) ability to build therapeutic alliance, (2) communicate emotions and feeling empathy via an online platform, as well as concerns about the (3) suitability of telemental health methods for certain types of clientele. Aims. Our study aimed to examine therapists’ experiences with these three common areas of concern after participating in an online, therapist-assisted intervention. Method. The RESILIENT intervention was developed to promote resilience after a natural disaster. It provides affected individuals a free access to a 12-module program that focuses on improving posttraumatic stress, insomnia, and depression symptoms. We conducted focus groups to collect data from participating therapists, and a thematic analysis for the qualitative analysis.Results. We found that therapists were pleasantly surprised by the ease of developing a strong alliance with their clients, and they felt that the online platform did not hinder their ability to perceive the patient’s emotions and feel empathy towards them. At the same time, they identified unique advantages (e.g. being more at ease than in person) and disadvantages (e.g. lack of nonverbal cues) compared to face-to-face therapy. Finally, the web-based intervention was judged to be less suitable for a severe symptomatology and when there is a discrepancy between patient’s expectations and the intervention. The patient’s engagement in the program appeared to be a facilitator of alliance, communication, and ultimately, treatment efficacy. Conclusion. We propose several approaches to improve such interventions, and emphasize their unique advantages. We also propose to add online intervention specific knowledge and skills building into training of clinicians and other providers to improve attitudes towards telemental health and the ability to utilize it more efficiently, for more conditions, and in a wider range of mental health settings.
