Comparaison d’une intervention basée sur la pleine conscience et d’un programme de relaxation chez les enfants et adolescents Comparison between a mindfulness based intervention and a relaxation program in children and adolescents

 

La pleine conscience est une approche de plus en plus utilisée chez les enfants et les adolescents. Des recherches ont pu mettre en évidence son intérêt clinique dans ces populations. Cependant, les mécanismes psychologiques spécifiques à ces interventions restent encore incertains. Cette étude a pour objectif premier de comparer l’efficacité d’un groupe de relaxation à un groupe de pleine conscience sur la symptomatologie dépressive et anxieuse.  Elle vise aussi à identifier les processus psychologiques qui seraient spécifiques aux interventions basées sur la pleine conscience (IBPC). Un groupe de 15 adolescents a participé à un programme de relaxation et un groupe de 17 adolescents a participé à une IBPC. Le niveau de dépression, d’impulsivité, de pleine conscience et les aspects métacognitifs tels que les ruminations et la fusion cognitive/évitement expérientiel ont été mesurés pour les deux groupes avant et après l’intervention. Les résultats indiquèrent que l’IBPC semblait jouer un rôle dans les processus métacognitifs alors qu’aucune des deux interventions n’a permis de réduire le niveau de la symptomatologie dépressive. Cette étude démontre la nécessité de poursuivre les investigations concernant les IBPC dans les populations d’enfants et d’adolescents.

 

Mindfulness is a new approach increasingly used in children and adolescents. Numerous studies have shown the clinical interest of mindfulness in these populations. However, the psychological mechanisms specific to this kind of interventions are still equivocal. The first objective of this study is to compare the efficacy of a relaxation group with a mindfulness group on depressive and anxious symptomatology. The second objective is to identify the specific psychological processes of mindfulness based interventions (MBIs). A group of 15 adolescents participated in a relaxation program while a group of 17 adolescents participated in a MBI. Levels of depression, impulsivity, mindfulness and metacognitive aspects such as ruminations and cognitive / experiential avoidance were assessed for both groups before and after the intervention. The results indicated that the MBI had an effect on metacognitive processes, whereas none of the interventions reduced the level of depressive symptomatology. This study demonstrates the need to continue investigating MBIs in the populations of children and adolescents.

Etude de cas : réduction de l’anxiété dans le cadre de troubles délirants chez une patiente souffrant de schizophrénie et d’un retard mental Case study: CBT of a person with intellectual disability and presenting a schizophrenia

 

Le modèle biopsychosocial a été choisi pour cette prise en charge. Cette prise en charge a débuté d’abord par une intervention pharmacologique couplée à une intervention socio-éducative. Lorsque la patiente a été stabilisée, il était important de faire un travail sur les manifestations de stress et d’anxiété qui entourent les idées délirantes. C’est pourquoi l’intervention des TCC a été retenue. 21 séances hebdomadaires ont été nécessaires pour réduire les manifestations de stress et d’anxiété dont 10 séances pour construire l’alliance et récolter des données anamnestiques. Une séance a été dédiée à l’analyse fonctionnelle et notre choix s’est porté sur une analyse synchronique (c’est-à-dire transversale) dans l’ici et maintenant. Nous avons également effectué une liste des situations anxiogènes. L’intelligence et les capacités cognitives de la patiente étant déficitaires, la mise en place de la thérapie a subi quelques aménagements.

The Biopsychosocial model was chosen for this case. The patient care began with a pharmacological treatment coupled with a socio-educational intervention. When the patient was stabilised, we worked on the stress and anxiety that surrounding delusional ideas. This is why the cognitive behavioural therapy (CBT) was chosen. Twenty-one weekly sessions were required to reduce the manifestations of stress and anxiety, including 10 sessions to build the alliance and collect anamnestic data. A session was dedicated to a functional analysis and our choice was a synchronic analysis (i.e. Transversal). We also made a list of anxiety-provoking situations. Due to the patient’s intelligence and cognitive disabilities, the setting of the therapy had had some modifications.
